You will need to bring these packets with you to your first session so either fill out the digital version or download and print the forms and bring them with you. Please include each individual’s name on the forms. For example if Cathy is going to be seeing your child(ren), please list your name as well as your child(ren)’s name(s) on each line asking for patient name. Please fill out or download the following forms if you are going to be seeing Cathy L. McGinnis, LCSW-S, RPT-S.

*NOTE: Please call our office manager, to schedule your initial session. We will need to get an intake on you before we see you for your first visit. Please bring a COPY of your entire divorce decree, temporary orders or associate judge’s report to your initial session for your file (if you are a parent who is separated, divorced or going through a custody battle). When seeing children whose parent’s are divorced, a final divorce decree must be on file to ensure that the parent bringing the child has the right to obtain psychological treatment for the child.
