Please complete the entire CONTRACT and CONCENT FOR TREATMENT packet. You may print the forms or bring them with you to your first session and/or fill out the digital version. If Cathy is seeing your child(ren), please include each individual’s name on the forms. For example, if Cathy is going to be seeing your child(ren), please list your name as well as your child(ren)’s name(s) on each line asking for the patient name.

*NOTE: Please call our office manager, to schedule your initial session. We will need to get a short intake before we see you for your first visit. Please bring a COPY of your entire divorce decree, temporary orders or associate judge’s report to your initial session (if you are a parent who is separated, divorced or temporary orders if the divorce is not yet final. When seeing children whose parent’s are divorced, a final divorce decree MUST be on file. If you do not bring a copy of your legal documents, there will be a charge for making copies for our files.
